Description

PURPOSE: The Albuquerque Area Indian Health Service (AAIHS), Santa Fe Indian Health Center (SFIHC) & Satellite Clinics (specified below) has a requirement for Pest Control Services, Santa Fe, NM 87505. Description of Services: The Albuquerque Area Indian Health Service requires a contractor to provide comprehensive Integrated Pest Management for the Santa Fe Indian Health Center and the San Felipe Health Clinic. Services include but are not limited to: Routine Integrated Pest Management (IPM) inspections and monitoring Interior and exterior pest control services Rodent control and monitoring Structural pest management, including termites and other wood-destroying organisms Wildlife and nuisance animal management (where permitted by law) Emergency pest response services Bed bug inspection and treatment (as required) Lice management services (as required) Installation, inspection, and maintenance of traps, bait stations, and monitoring devices Application of EPA-registered pesticides in accordance with all applicable Federal, State, Tribal, and local regulations Inspection reports, service documentation, and pesticide application records Recommendations for corrective actions and Integrated Pest Management improvements Monthly service reports and other contract deliverables identified in the Statement of Work (SOW) The Contractor shall provide all management, supervision, personnel, labor, materials, equipment, tools, vehicles, pesticides, permits, licenses, transportation, and all other resources necessary to perform the required services. No Government-furnished equipment, materials, or pest control supplies will be provided. WORK SCHEDULE: Facility Hours of Operation: Santa Fe Indian Health Center: Monday-Friday, 8:00 AM 6:00 PM Maintenance Building: Monday-Friday, 8:00 AM 5:00 PM Business Office Building: Monday-Friday, 8:00 AM 5:00 PM OEHE Office Building: Monday-Friday, 8:00 AM 5:00 PM Dental Building: Monday-Friday, 8:00 AM 5:00 PM San Felipe Health Clinic: Monday-Friday, 8:00 AM 4:00 PM Services shall be coordinated to minimize disruption to patient care, clinical operations, and administrative activities. PLACE OF PERFORMANCE: (CLINICS ARE UNDER THE SANTA FE INDIAN HEALTH CENTER) Santa Fe Indian Health Center (Primary Health Center) Attn: Administration 1700 Cerrillos Road Santa Fe, NM 87505 San Felipe Health Clinic (Satellite Clinic) 04 Cedar Street San Felipe Pueblo, NM 87001 The anticipated service locations include: Santa Fe Indian Health Center 84,025 SF Maintenance Building 1,800 SF Business Office Building 3,920 SF Dental Building 3,067 SF OEHE Office Building 3,196 SF San Felipe Health Clinic 2,440 SF Total Estimated Service Area: Approximately 98,448 square feet. Square footage is provided for market research purposes only and is subject to change. The final solicitation will identify the official scope of work and performance requirements. Utilize the information received with future acquisition planning efforts and potential Buy Indian set aside. HOW TO RESPOND: This is a sources sought notice under the authority of the Buy Indian Act. The Indian Health Service (IHS) is attempting to locate qualified and eligible Small Business Indian Firms, Native American Indian Owned and Operated businesses, that can provide services/supplies to the Indian Health Service. This is a request for information only, this is not a solicitation. Responses to this notice are not considered offers and cannot be accepted by the Government to form a binding contract. Any information submitted by respondents to this notice is strictly voluntary with no obligation or incurred cost by the government. Provide with response Certification as a Small Business Indian Firm. The government is ONLY interested in Small Business Indian Firms. (See attached Representative Form). Provide Unique Entity Identifier (UEI) created in SAM.gov registration, in accordance with FAR 4.1102(a). Prospective vendors business size for NAICS code listed, including Indian Small Business Economic Enterprise (ISBEE); HUBZone; Service Disabled Veteran Owned, Veteran Owned (SDVOSB); Small Disadvantage Business (SDB); Women-Owned, Small Disadvantaged (WOSB). Provide detailed capabilities statement, website, and e-catalog as it pertains to 561710 Exterminating and Pest Control Services. GSA/FSS contract holders provide GSA/FSS/VA or other GWAC contract number, only if applicable.
