Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR Part 12. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This requirement is for one (1) Firm-Fixed-Price (FFP) purchase order for contracted Certified Medical Assistant (CMA) and Licensed Practical Nurses (LPN) to support the Medical Program at the Warm Springs Service Unit (WSSU), Warm Springs, Oregon. The purpose of this requirement is to provide supplemental professional medical and nursing to ensure continuity of patient care services and access to care. The contracted CMA or LPN shall assist in maintaining safe, accurate, and effective dental services while supporting ongoing clinical operations and compliance with applicable standards of care. Attachment No. 1 Pricing Schedule, (Supplies or Services and Prices/Costs) Attachment No. 2 Wage Determination Attachment No. 3 Business Associate Agreement Attachment No. 4 IEE Form PERFORMANCE WORK STATEMENT DENTAL PROVIDER AND DENTAL HYGIENIST SERVICES WARM SPRINGS SERVICE UNIT (WSSU) I. BACKGROUND The Warm Springs Health and Wellness Center s mission is to provide an innovative, caring, and diverse healthcare system that is trusted and responsive to the needs of the community. The Warm Springs Health and Wellness Center is a primary care clinic that provides quality healthcare to all eligible American Indians and Alaska Natives in the Warm Springs Service Unit Area. The Warm Springs Health and Wellness Center provides a comprehensive system of healthcare services including medical, dental, optometry, pharmacy, laboratory, radiology, podiatry, and community health services. The medical program emphasizes prevention, patient education, clinical support, and comprehensive family-oriented dental treatment. Due to limited Purchase/Referred Care (PRC) resources and continued community demand for Nursing services, the Warm Springs Service Unit requires contracted Certified Medical Assistant (CMA) and Licensed Practical Nurse (LPN) services to improve access to care and support continuity of patient care. II. OBJECTIVE The objective of this contract is to provide clinical nursing and medical support services for eligible patients of the Warm Springs Service Unit. Services shall include patient assessment, vital signs, clinical support, patient education, care coordination and related medical support services. The contractor shall provide professional CMA and/or LPN services up to five (5) days per week as coordinated with the Contracting Officer Representative (COR). III. PERIOD OF PERFORMANCE AND LOCATION A. Location Services shall be performed at: Warm Springs Health and Wellness Center 1270 Kot-Num Road Warm Springs, OR 97761 B. Period of Performance The period of performance shall be one year from date of award. C. Clinic Schedule Clinic hours are generally: Monday, Tuesday, Thursday, Friday: 8:00 AM 5:00 PM Wednesday: 12:00 PM 5:00 PM The contractor shall be entitled to a one-hour lunch period. The clinic is closed on federally recognized holidays and during emergency or administrative closures. IV. PERFORMANCE-BASED REQUIREMENTS A. Performance Objectives The Contractor shall provide comprehensive performance-based CMA and LPN services that improve access to care, maintain continuity of care, support preventive healthcare initiatives, and ensure high-quality clinical outcomes for eligible beneficiaries of the Warm Springs Service Unit (WSSU). The Contractor shall achieve measurable outcomes consistent with Indian Health Service (IHS) standards, Accreditation Association for Ambulatory Health Care (AAAHC) requirements, accepted standards of nursing and clinical practice, and WSSU operational goals. Performance shall be evaluated based on quality of care, patient access, documentation accuracy, professional conduct, clinical productivity, compliance with regulatory requirements, and timely completion of services. B. Performance Standards The Contractor shall: Maintain timely and accurate clinical documentation in RPMS/EHR; Provide nursing care services in accordance with accepted professional standards; Maintain compliance with infection prevention and control standards; Demonstrate professionalism and cultural sensitivity in all patient interactions; Meet scheduled clinic coverage requirements; Coordinate continuity of care and referrals; Comply with HIPAA, Privacy Act, and all applicable federal requirements; Maintain active licensure, certifications, and credentialing throughout the contract period. Performance deficiencies identified by the Government may require corrective action plans and may impact continuation of services. V. Tasks A. General Requirements The contractor shall provide direct patient care services in a responsible, respectful, professional, and caring manner consistent with accepted standards of nursing practice and Accreditation Association for Ambulatory Health Care (AAAHC) standards. The contractor shall: Interview and assess patients; Review of medical histories; Perform nursing assessments and evaluations; Record findings and nursing care provided in the electronic health record system (RPMS/EHR); Provide nursing care and respond to urgent/emergency patient needs within the contractors of practice; Coordinate referrals when additional medical or specialty care is required; Provide patient education and health promotion/prevention services; Maintain compliance with HIPAA and all applicable federal regulations; Utilize RPMS/EHR, Microsoft Office, Outlook, and other required systems; Collaborate with medical providers, nursing staff, and community health programs. The contractor shall document all patient care in accordance with WSSU policies and standards. B. CMA Services 1. CMA Task Requirements The CMA Contractor shall independently perform professional medical assisting services within the scope of granted clinical privileges and certification or registration. CMA responsibilities include, but are not limited to: Interviewing and obtaining patient information; Reviewing and evaluating medical histories and current health status; Performing clinical support activities within scope of practice; Obtaining and recording Vital signs withing scope of practice; Assisting providers with examinations, procedures, and patient care; Performing clinical procedures within scope of practice and applicable orders; Providing patient education and health promotion services; Preparing patients for examinations and procedures; Collecting and preparing specimens as authorized and within scope of practice; Coordinating referrals and follow-up care; Providing patient support and follow-up care; Documenting all patient encounters and clinical support services in RPMS/EHR; Collaborating with medical providers, nursing staff, and other clinical and support staff as appropriate; Ensuring continuity of patient care. 2. CMA Performance Outcomes The CMA Contractor shall: Deliver comprehensive medical assisting services consistent with accepted standards of medical assisting practice; Maintain accurate and timely patient records; Demonstrate professional clinical judgment within scope of practice; Maintain productivity and clinic efficiency; Ensure compliance with infection control standards; Provide culturally appropriate patient care; Coordinate continuity of care and referrals effectively. C. LPN Services 1. LPN Task Requirements The LPN Contractor provides preventive, clinical, and therapeutic nursing services within the scope of state licensure and granted privileges. LPN responsibilities include, but are not limited to: Performing patient health assessments and risk assessments as permitted within scope of practice; Reviewing and documenting patient health history; Recording blood pressure and applicable screening information; Performing appropriate health screenings and assessments; Completing nursing assessments and monitoring patient health status; Performing nursing interventions and patient care within scope of practice; Provide medication administration and management within scope of practice and applicable orders; Providing patient education and preventive health education; Counseling patients regarding health, wellness, and disease prevention; Coordinating follow-up care and preventive care scheduling; Administering medication and treatments as authorized and within scope of practice; Performing nursing procedures within licensure and granted privileges; Assisting with post-procedure care and follow-up as authorized; Documenting all patient encounters and nursing services in RPMS/EHR; Collaborating with medical providers, nursing staff and other healthcare providers regarding patient care. 2.
