AMENDMENT TO COMBINED SYNOPSIS/SOLICITATION The purpose of this amendment is to respond to questions received from prospective offerors and to amend the solicitation accordingly. Attachment No. 1 is hereby deleted in its entirety. Attachment No. 2 has been revised to include option periods and is hereby incorporated into the Combined Synopsis/Solicitation. Quoters shall submit quotes using the revised Attachment No. 2 provided with this amendment. 1. Question: Could the Government please clarify whether the five-year ordering period consists of a base period with option years or a continuous five-year ordering period? Answer: The period of performance is five (5) years, including option periods. 2. Question: Could the Government please clarify how frequently the Public Health Nurse will be expected to perform community outreach or home visits? Answer: The frequency of community outreach and home or field visits will vary based on patient needs, communicable disease activity, public health priorities, and scheduled community events. These activities will be performed as needed during scheduled duty hours. The Government does not guarantee a specific number or frequency of outreach activities or home visits. 3. Question: Is there a current incumbent contractor? If so, please provide the contract number. Answer: No. There is no current incumbent contractor; therefore, no contract number is applicable. 4. Question: Is on-campus housing available for the Registered Nurse and Public Health Nurse? If not, where do providers typically stay, and are there nearby hotels available? Answer: No. On-campus housing is not available. Private rental housing may be available in Toppenish, Zillah, Sunnyside, Yakima, and other nearby communities. Commercial hotels, including extended-stay lodging, may also be available in Yakima and the surrounding area. The Government does not guarantee lodging availability or endorse any particular property or lodging provider. 5. Question: Could the Government please identify the locations where community outreach events will be conducted within the Yakama Nation Reservation? Answer: Outreach activities may occur throughout the geographic area served by the Yakama Service Unit. Common outreach locations include Toppenish, White Swan, Wapato, Goldendale, and communities along the Columbia River. Specific locations will depend on public health needs and will be coordinated with the contractor in advance to the extent practicable. 6. Question: Could the Government please provide the anticipated contract award date and expected performance start date? Answer: The Government anticipates making an award prior to the end of Fiscal Year 2026 (September 30, 2026). Contractor personnel shall not begin performance until all applicable credentialing, fingerprinting, background investigation, orientation, and facility access requirements have been successfully completed. 7. Question: Could the Government please clarify whether transportation or a Government vehicle will be provided for Public Health Nurses when conducting community outreach or patient home visits? If not, will contractor personnel be expected to use their personal vehicles for official travel? Answer: Yes. When contractor Public Health Nurse personnel are required to travel for authorized community outreach or patient home visits, transportation will be provided by Government-employed Public Health Nurse personnel. Contractor personnel will travel as passengers and will not be authorized to operate Government-owned or GSA Fleet vehicles. Contractor personnel will not be required or expected to use personally owned vehicles for authorized community outreach or patient home visits. Contract Title Registered Nursing and Public Health Nurse Support Services (RN & PHN) Yakama Indian Health Center (YIHC) II. Background The Portland Area Indian Health Service (PAIHS), Yakama Indian Health Center (YIHC), located in Toppenish, Washington, is the principal federal health care provider and health advocate for American Indian and Alaska Native patients within its service area. YIHC provides primary care and urgent care services Monday through Friday, 8:00 a.m. to 5:00 p.m. To meet operational needs and ensure continuity of patient care, YIHC requires contractor-provided nursing services performed by licensed registered nurses (RNs). These services support the delivery of safe, culturally competent, and comprehensive healthcare to eligible patients. This will be a Firm-Fixed Price (FFP) purchase order. This contract provides for up to 1 Registered Nurse and 1 Public Health Nurse each year, not to exceed 1,920 total labor hours annually III. Period Of Performance and Location: a. Services shall be performed (1) on-site at the Yakama Indian Health Center (YIHC), 401 Buster Road, Toppenish, WA 98948 and (2) at community outreach events within Yakama Nation Reservation, WA. b. The Contractor shall provide qualified Registered Nurse (RN) to perform non-personal nursing support services at the Yakama Indian Health Center. The RN shall assist Government medical staff in providing direct patient care, care coordination, and clinical support activities in accordance with professional nursing standards and YIHC policies. All services shall be monitored by Contracting Officer s Representative (COR), consistent with a non-personal services contract. 2. The services will be provided during clinic operating hours as scheduled not to exceed 40 hours per week. Regular hours of clinic are from 8:00 a.m. to 5 p.m. Monday through Friday. The clinic is not open on federally recognized holidays or certain day(s) of closure pursuant to an Executive Order by the President of the United States. Inclement weather conditions or facility issues (electrical, water, etc.) may also hinder the normal operating hours and notice of closure will be provided as soon as is practicable by each of the Service Unit administrative staff. 3. This Performance Work Statement (PWS) applies to two distinct Contract Line Item Numbers (CLINs) under the Yakama Indian Health Center (YIHC) Nursing Support Services contract. The Government guarantees a minimum order of 200 labor hours per CLIN over the life of the contract 4.
