Amendment 0001 is hereby issued to Revise Evaluation 1 Factor: Technical Capability, CLIN 4 Description with Quantity, CLIN 17 Quantity (Attachment B), and to extend the due date to Friday, August 21, 2026 by 04:00 pm MDT. _________________________________________________________________________ **THIS IS AN INDIAN-OWNED SMALL BUSINESS ECONOMIC ENTERPRISE (ISBEE) SET-ASIDE** The Indian Health Service, Kayenta Health Center located at Highway 160 Mile Post 394.3, Kayenta AZ 86033 has a requirement for providing Chemical, Biological, Radiological, and Nuclear (CBRN) respiratory protection equipment and hazardous material (hazmat) response supplies. The Government will award a contract on a Best Value Trade-Off basis to the responsible offeror whose quotation represents the best overall value to the Government, price and non-price factors considered. The Government will evaluate quotations based on three primary factors, listed in descending order of importance: Technical Capability and Demonstrated Experience (Most Significant) Price Delivery Schedule (Least Significant) As the technical differences between proposals decrease, the importance of price in the final award decision will increase. The Government reserves the right to award to other than the lowest-priced offeror if the superior technical capability or qualifications of a higher-priced offeror warrants the additional cost. Offerors must strictly follow the submission instructions detailed in Section E (page 20) of this solicitation to be considered for evaluation and award. All technical and administrative questions must be submitted via email no later than Wednesday, August 5th at 5:00pm EDT. Questions received after this deadline may not be answered. Please ensure all quotations are prepared in strict accordance with these instructions. Failure to provide any of the required documentation shall render the quotation incomplete and ineligible for award without further evaluation.
