COMBINED SYNOPSIS/SOLICITATION INTEGRATED PEST MANAGEMENT (IPM) SERVICES This is a combined synopsis/solicitation for commercial services prepared in accordance with FAR Part 12 and the applicable Revolutionary FAR Overhaul (RFO) class deviation. This announcement constitutes the solicitation and quotations are being requested in response to this notice. The Department of Health and Human Services (HHS), Indian Health Service (IHS), Albuquerque Area Office (AAO), is soliciting quotations for comprehensive Integrated Pest Management (IPM) services for the Santa Fe Indian Health Center and San Felipe Health Clinic. Solicitation Number: 75H70726Q00086 Solicitation Type: Request for Quotation (RFQ) Set-Aside: Indian Small Business Economic Enterprise (ISBEE) NAICS Code: 561710 Exterminating and Pest Control Services Contract Type: Firm-Fixed-Price (FFP) Period of Performance: One (1) base year and four (4) one-year option periods, with the potential extension of services for up to six (6) months pursuant to FAR 52.217-8. The Contractor shall provide all labor, supervision, personnel, equipment, materials, pesticides, transportation, licenses, permits, monitoring devices, reporting, and other resources necessary to provide comprehensive IPM services in accordance with the attached Statement of Work (SOW). Services will be performed at the following locations: Santa Fe Indian Health Center 1700 Cerrillos Road Santa Fe, NM 87505 San Felipe Health Clinic 04 Cedar Street San Felipe Pueblo, NM 87001 This acquisition is set aside for eligible Indian Small Business Economic Enterprises (ISBEEs). Quoters shall complete and submit the required IHS IEE Representation Form with their quotation. BASIS FOR AWARD The Government intends to award one (1) Firm-Fixed-Price contract to the responsible quoter whose quotation is technically acceptable, has acceptable past performance, and offers the lowest evaluated price determined fair and reasonable. The Government will evaluate quotations using the following factors: Technical Capability Past Performance Price Technical Capability and Past Performance will be evaluated on an Acceptable/Unacceptable basis. Price will be evaluated for completeness and accuracy and to determine whether the quoted price is fair and reasonable. QUOTATION SUBMISSION Quoters shall submit: Volume I Technical Capability Volume II Past Performance Volume III Completed Pricing Schedule Completed IHS IEE Representation Form Any other required representations and certifications identified in the solicitation Questions regarding this solicitation shall be submitted by the date and time identified in the solicitation. Quotations shall be submitted electronically by the response date and time identified in this SAM.gov notice and the attached solicitation. Quoters are responsible for reviewing the complete solicitation and all attachments prior to submitting a quotation. The attached solicitation, Statement of Work (SOW), Quality Assurance Surveillance Plan (QASP), applicable Service Contract Labor Standards Wage Determinations, and other attachments contain the complete requirements of this acquisition.
