AMENDMENT 0001: The purpose of this amendment is to extend the due date to 21 July 2026 at 10:00 am (HST). *************************************************************************************************************************************************** This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in RFO Part 12, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Defense Health Agency Contracting Activity, Healthcare Contracting Division West, Foxtrot Branch, Bldg 339 146 Clark Road, Ft Shafter, HI 96858, intends to solicit, negotiate, if necessary, and award firm fixed-price contract for a water purification system, reverse osmosis, hemodialysis for the Brian D. Allgood Army Community Hospital, Korea. This is a request for quotation (RFQ). The solicitation number is HT941026QE020. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2026-01. The Standard Industrial Classification Code is 4610, size standard is 1000 employees and the NAICS code of 333310. NOTICE TO QUOTERS: All sources may submit a quote as applicable. To comply with the US-ROK Status of Forces Agreement (SOFA) and USFK Regulation 715-2 (Local Sourcing Preference), the Government will evaluate quotes using a Tiered Evaluation process. Offerors must prominently state on the cover page of their quote whether they are submitting as a Tier 1 (Korean Local Source) or a Tier 2 (U.S./Off-Shore Source). Failure to clearly identify the appropriate Tier may result in the quote being deemed non-responsive. Provision at 52.212-1, Instructions to Offerors -- Commercial, and its addendum applies to this acquisition and a statement regarding any addenda to the provision. Provision at 52.212-2 Evaluation - Commercial Items and its addendum applies to this acquisition. Offerors shall include a completed copy any provisions specified in the combined synopsis/solicitation attachment. The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, and its addendum applies to this acquisition. All applicable provisions and clauses are in the combined synopsis/solicitation attachment. The Defense Priorities and Allocations System (DPAS) and assigned rating is not applicable. The Government intends to award a firm fixed-price contract to the successful quoter.
