AMENDMENT 2 TO COMBINED SYNPOSIS/SOLICITATION This amendment is issued to provide response to interested vendors' inquiries and update the email address for the submission of quotations to PORAOAcquisition@ihs.gov. Attached is Attachment 5 - Questions and Responses. AMENDMENT 1 TO COMBINED SYNPOSIS/SOLICITATION This amendment is issued to implement the requirements of EO 14398, Addressing DEI Discrimination by Federal Contractors. FAR 52.222-90, Addressing DEI Discrimination by Federal Contractors (APR 2026) (DEVIATION APR 2026) applies to this acquisition. This is a combined synopsis/solicitation for commercial services prepared in accordance with RFO FAR Part 12. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Solicitation No. 75H71326Q00036 has been issued as a Request for Quotation (RFQ) for a firm-fixed-price commercial services purchase order for Medical Waste Disposal Services for the Indian Health Service, Portland Area Office, Western Oregon Service Unit, Salem, Oregon. The scope of work includes routine regulated medical waste collection and disposal services for the Western Oregon Service Unit, including compliant containers and liners, a secure exterior biohazardous waste storage cabinet, scheduled monthly pickups, one additional high-volume pickup per contract year, transportation, treatment/disposal, manifests, and disposal documentation. The anticipated period of performance includes one 12-month base period and four 12-month option periods. The base period shall begin within 30 calendar days after award and continue for 12 months. The actual start date and end date will be stated in the resulting award. Each option period, if exercised, will continue for 12 months following the prior performance period. The place of performance is the Western Oregon Service Unit campus, Chemawa Indian Health Center, 3750 Chemawa Road NE, Salem, Oregon 97305. Government acceptance will occur at the Western Oregon Service Unit. The Government may consider a quotation received after the due date and time only if the Contracting Officer determines that doing so would not unduly delay the acquisition and would be in the Government interest. Quotes shall include: Completed price quote (CLIN Schedule of Items) with annual fixed prices for the base period and all four option periods, and shall include all labor, management, supervision, containers, liners, secure exterior storage cabinet, equipment, tools, materials, transportation, treatment/disposal fees, manifests, regulatory documentation, insurance, overhead, profit, and other costs necessary to perform the requirement.
