*****AMENDMENT 0001***** The purpose of this amendment is to correctly identify the NAICS code and size standard as: 339112 Surgical and Medical Instrument Manufacturing Size Standard: 1,000 Employees A copy of the amendment is available for you to download. Please be sure to provide acknowledgement of the amendment with your quote. *****AMENDMENT 0001***** ORIGINAL SYNOPSIS: The Dayton VAMC has a requirement for one Drying Cabinet Package (Pass-Through), Cenorin Model 350 with LD200 Lumen Drying System (Or Equal); and one Drying Cabinet Package (Non-Pass-Through), Cenorin Model 350 with LD200 Lumen Drying System (Or Equal). Each package includes the 350-model drying cabinet, 15 half-width shelves, drip tray, a tube drying rack, and a HEPA filtration system. Each LD200 Lumen Drying System includes a right-side user interface with robotic surgical device holder, pump housing, and universal scope holder. The Dayton VAMC also requires two each Trim Kit Set for the 350 Series Drying Cabinet (Or Equal) that prevent crossover contamination from our decontamination area to prep area and two each installation (one for each cabinet) at the Dayton VAMC. SALIENT CHARACTERISTICS: For those vendors quoting an Or Equal part, the Request for Quotation (RFQ) states the salient physical, functional, or performance characteristics of the brand name item that an equal item must meet or exceed to be acceptable for award. SDVOSB SET-ASIDE: This solicitation is set-aside for those concerns SBA-certified as Service-Disabled Veteran-Owned Small Business (SDVOSB). Please see clause VAAR 852.219-73 VA Notice of Total Set-Aside for Certified Service-Disabled Veteran-Owned Small Businesses (JAN 2023) (Deviation) in the solicitation. IMPORTANT NOTICE AND INSTRUCTIONS: Complete the certification in clause VAAR 852.219-76 - VA Notice of Limitations on Subcontracting Certificate of Compliance for Supplies and Products and return it with the quote. The ENTIRE clause MUST be included with your quote. Failure to return the ENTIRE clause at VAAR 852.219-76, containing a fully and properly completed certification by the time quotes are due, shall result in the offer being rejected and deemed ineligible for evaluation and award.
