The Government hereby submits a notice of intent to award a sole source, firm fixed price purchase order to State of Arizona, under NAICS code 621511 Testing laboratories, medical, by the Phoenix Area Office (PAO) Indian Health Services (IHS) under the authority of RFO FAR 6.103-1(b), One responsible source, because only one source is reasonably available to satisfy the Government's requirement. The requirement is for a contractor to provide for newborn screening laboratory testing services for the Whiteriver Service Unit (WRSU) and Cibecue Health Center. This is NOT a request for quotes or proposals.
