Responses to this notice will be used for information and planning purposes. No quotes are being requested or accepted at this time with this notice. The Department of Veterans Affairs, VA Medical Center Wilmington at 1601 Kirkwood Hwy Wilmington, DE 19805 has a requirement for Preventive Maintenance Service Agreement for the Medical Lasers in use at the facility The North American Industry Classification System (NAICS) code being considered for this procurement is 811210 Electronic and Precision Equipment Repair and Maintenance, (PSC) is J065 Electronic and Precision Equipment Repair and Maintenance. Responses to this notice should include the following: Company name SAM Unique Entity Identifier (UEI) Company s address, Point of Contact information (i.e. title, phone number and email address) Company s capability to meet this requirement and any pertinent information which demonstrates the company s ability to meet the above requirements. Supporting evidence must be furnished in sufficient detail to demonstrate the ability to perform the requirements and must include relevant corporate experience along with the applicable contract number, total contract dollar amount, contract period of performance, details of the relevant services and customer point of contact with corresponding telephone number and email address.GSA Federal Supply Schedule contract number or other applicable contract vehicle contract number, if within scope of this effort (if applicable). All responses shall be submitted in writing via email no later than 4:00 PM (ET) August 17, 2026 to Derek Chuff Derek.Chuff@va.gov . Statement of Work Description: Preventive Maintenance Service Agreement for Medical Lasers INTRODUCTION/BACKGROUND: The Wilmington VAMC wishes to procure a Preventive Maintenance Service Agreement for the Medical Lasers in use at the facility. OBJECTIVE: The Wilmington Veterans Affairs Medical Center, (460) Wilmington, Delaware, hereby referred to as the VAMC Wilmington (460), is seeking to purchase this service agreement for the following: 1 PM Lumenis Sharplan 30C #41033 2 PM Lumenis Versapulse Select #1884311507 3 PM Lumenis Verapulse Poweresuite 006-11 (twice a year) 4 PM Oculight GLX #GL15174C30 5 PM Lumenis Selecta II #001-51184 6 PM Lumenis Pulse 120H #432 (twice a year) 7 PM Iridex IQ532 #IQG1344C 8 PM IridexCyclo G6 #IQ192110C 9 PM Ellex Tango #TG4645 10 Iridex IQ532 #AE6000458 MAJOR REQUIREMENTS: The Contractor/ Vendor must complete the following tasks to meet the needs of this acquisition: Perform Preventive Maintenance Service on the 10 Lasers listed above Shall comply with all applicable records management laws and regulations. (Please refer to Records Management Language for Contracts) Coordinating service with Biomedical Engineering and check-in at arrival and departure. Service to be provided at: Wilmington VA Medical Center 1601 Kirkwood Hwy Wilmington, DE 19805 GOVERNMENT FURNISHED INFORMATION OR MATERIAL: This acquisition does not include Government-furnished property (GFP), identify in the Statement of Work all available information on GFP to be provided, including the make, model, bar code number and serial number, if available, of all accountable assets with an acquisition cost of $500 or more. This acquisition does not include Contractor-acquired property (CAP).
